ICAM software for incident investigation.

InvestigatePro is web software for running ICAM investigations, the Incident Cause Analysis Method. It covers the whole case: PEEPO evidence, the sequence of events, all four ICAM levels, actions on the hierarchy of controls, and a report that stands up to a board or a regulator.

InvestigatePro at a glance

Key facts about InvestigatePro
Who it is forSafety, HSE and SHEQ leads at industrial businesses under 1,000 people. That covers mining and quarry operators, mining contractors (drilling, blasting, geotech, ventilation, plant and labour hire), oil and gas, energy, manufacturing, engineering and construction.
MethodICAM, run as designed. Evidence is gathered under PEEPO (People, Environment, Equipment, Procedures, Organisation), then analysed across the four levels: absent or failed defences, individual or team actions, task or environmental conditions, and organisational factors.
What you end up withA signed investigation report (PDF or PPTX) with the evidence, timeline, contributing factors at every level, and recommendations on the hierarchy of controls. Every action is tracked until someone checks and verifies it.
The coachOn Professional and above, a coach works inside the live case. It flags evidence gaps, suggests witness questions and prompts any ICAM level left empty. The investigator accepts, edits or rejects every suggestion.
PriceStarter $149/month for up to 5 users (no coach). Professional $349/month for up to 20. Business $749/month for up to 100. Enterprise from about $1,500/month. One price per organisation, no per-seat charges.
How to startStarter and Professional: a self-serve 14-day trial. Business and Enterprise: a 15-minute demo, then a guided pilot on your own cases.
Where it sitsAlongside your existing EHS platform, not in place of it. That platform stays your system of record; InvestigatePro is where the investigation gets done, and its report and actions feed back into it.
PlatformRuns in a web browser. Mobile apps are in build and not yet released.

What ICAM software should do.

Use this as a checklist for any tool you evaluate, ours included. A form that has the four ICAM headings on it is not the same as software that runs the method.

Criteria for ICAM investigation software and how InvestigatePro meets each
What to checkWhy it mattersHow InvestigatePro does it
Evidence comes before analysisFindings that rest on no evidence fall apart at the first challenge.A PEEPO evidence register and a sequence of events, with photos, statements and documents attached, all built before any cause analysis starts.
All four ICAM levels are workedInvestigations that stop at individual actions blame the person and miss the system. The analysis covers every level, and the coach points out any level that has been left empty.
Every factor is tied to its evidenceA board or regulator will ask how you know. The coach cannot suggest a cause unless there is a record behind it, and it says so when it is not sure.
Actions sit on the hierarchy of controlsRetraining and reminders are the weakest controls, and the easiest to write down. Each recommendation is placed on the hierarchy of controls, so it is obvious when a case leans on weak controls.
Closure means verifiedAn action marked done is not the same as a control that works. The action board has a verified column. Actions stay visible until they are checked.
The investigator stays the authorThe people who sign the findings need to own them.Nothing is filed until an investigator accepts it. The coach prompts; the person decides.
The report is ready to signReassembling a report by hand in a word processor is where findings go missing. One formatted PDF or PPTX from the case itself. Read a sample investigation report.

Templates, EHS suites or dedicated ICAM software?

There are three broad ways to run ICAM, and each fits a different organisation.

Three ways to run ICAM investigations compared
ApproachWhere it fitsWhere it struggles
Word or Excel templatesOne trained investigator and a few cases a year. Free to start: see our ICAM template.Quality depends on whoever fills it in. Evidence and actions live in separate files, and nothing gets checked before sign-off.
General EHS platformsLarge operators that need one register for incidents, audits, permits and compliance. The investigation module is usually a form. The method lives in the investigator’s head, not in the software.
Dedicated ICAM softwareTeams that want one method on every case, run by every supervisor, with output that feeds the system of record. It is a second tool next to your register, so the hand-over between the two needs agreeing up front.

If you run 5,000 people and want one suite for everything, a general EHS platform is the better buy. InvestigatePro is built for getting one investigation done properly this week, at a business that does not have a full-time investigations team.

Built by an investigator.

Devon Turner spent fifteen years leading mining safety across Africa and the Middle East, including regional fatal-risk management and the investigation of serious incidents. He built InvestigatePro so a supervisor on a contractor crew can run ICAM to the same standard as a specialist.

Questions about ICAM software.

More on the full FAQ and the pricing page. New to the method? Read the ICAM methodology guide.

What is ICAM software?

Software that runs the Incident Cause Analysis Method end to end: gathering evidence, building the sequence of events, analysing contributing factors across ICAM’s four levels, and turning findings into actions and a report. Good ICAM software enforces the method. A blank form with ICAM headings does not.

Does InvestigatePro replace our EHS platform?

No. Your EHS platform stays your system of record. InvestigatePro is where the investigation itself is done, and its report and actions feed into that platform.

Does the AI decide the causes?

No. The coach prompts for evidence gaps, witness questions and empty ICAM levels, and drafts factors tied to their source. It cannot suggest a cause without a record behind it, and nothing is filed until an investigator accepts it. Starter has no coach at all.

How is ICAM different from a 5 Whys or root-cause analysis?

ICAM assumes an incident has several contributing causes across the system, not a single one. It looks beyond individual actions to failed defences, task and environmental conditions, and organisational factors. A 5 Whys follows a single chain. Compare methods in ICAM vs SCAT.

Does it suit mining contractors?

Yes. That is the core use: drilling, blasting, geotech, ventilation, plant hire and mine-engineering contractors who must investigate to their principal’s standard without an investigations department of their own. See mining incident investigation.

Does it work for US MSHA and OSHA reporting?

The investigation method is the same wherever you operate, and the report supports what you file. The statutory deadlines still apply: MSHA notification within 15 minutes (30 CFR 50.10) and form 7000-1 within 10 working days, and OSHA within 8 hours for a fatality or 24 hours for hospitalisation, amputation or eye loss (29 CFR 1904.39).

How much does it cost?

Starter $149/month (up to 5 users, no coach), Professional $349/month (up to 20), Business $749/month (up to 100), Enterprise from about $1,500/month. Annual billing saves 15%.
