The person and the team.
- Physical state: Fatigue and sleep, illness or injury, impairment, physical capability
- Mental state: Stress and pressure, distraction, complacency, rushing
- Knowledge: Training, experience, hazard awareness
Environment
Where the work happened.
- Physical: Weather, lighting, noise, temperature, ventilation
- Workspace: Housekeeping, layout and access, ground conditions, congestion
- Time: Time of day, shift patterns, remote location
Equipment
What the work was done with.
- Design: Suitability for the task, ergonomics, human factors, fail-safe design
- Condition: Maintenance status, wear, known defects, pre-use checks
- Safety features: Guards, interlocks, emergency stops, adequate PPE
Procedures
How the work was meant to be done.
- Availability: The procedure exists, is accessible and is in a usable format
- Quality: Accurate, complete, clear and up to date
- Use: Known, trained, followed, practical and enforced
Organisation
The decisions behind all of the above.
- Systems: Risk assessment, change management, permits, contractor management
- Resources: Staffing, budget, time and schedules
- Culture: Safety priority, reporting, leadership commitment, competing pressures
Four questions to keep asking.
They take the analysis from the event to the system.
- When you find human errorAsk why the error was possible, and what system allowed it. Do not stop at blame.
- When a procedure was not followedAsk whether it was practical, whether there was time, and whether there was pressure to shortcut it.
- For every P, E, E and P factorAsk what organisational decision created the condition.
- Before you close the investigationAsk whether fixing this will actually prevent it happening again.
