Incident description
What happened: factual, in order, no conclusions yet.
Immediate cause
The direct action or condition that caused harm. It is not the cause in the system.
Absent or failed defences: barriers or controls that should have prevented harm but were missing, failed or bypassed.
Contributing factors (PEEPO)
The conditions that made the incident possible.
Causes
Causes sit in management systems, culture, resources or design. Human error is not a cause.
Corrective actions
Prefer elimination and engineering controls to administrative controls and PPE. Every cause needs at least one action.
