When an incident happens on your site, the quality of your investigation determines whether the same event happens again. This checklist gives your team a structured path through every phase — from the moment something goes wrong to verified corrective action close-out.

Phase 1: Immediate Response (0–30 Minutes)
The actions taken in the first 30 minutes after an incident determine the quality of everything that follows. Get these right.

Phase 4: Evidence Collection (Days 1–3)
Construction incidents typically involve multiple evidence types. Capture all of them systematically.

Photograph all physical evidence with a scale reference (ruler, hard hat, etc.)
Measure and sketch the scene — distances, heights, positions
Collect failed components for engineering assessment (scaffolding clips, harness hardware, tool parts)

Document equipment condition: age, maintenance status, last inspection date
Check equipment data recorders or telematics if applicable (cranes, elevated work platforms)

Collect the relevant SWMS (Safe Work Method Statement) or JSA for the task being performed

Phase 5: Witness Interviews
Witness interviews are often the most critical and most poorly-executed part of construction investigations. Follow these principles.

Interview witnesses individually — never in groups
Interview as soon as possible after the incident — memory degrades rapidly
Choose a private, neutral location away from the worksite
Explain the purpose: learning, not blame. Assure confidentiality where possible.
Start with open questions: "Tell me what happened from the beginning"
Let the witness give their full account before asking specific questions
Avoid leading questions — ask what happened, not "did you see him fall?"
Probe for: what was normal about the task, what was different that day
Ask about conditions: time pressure, fatigue, equipment issues, supervisor communication
Document responses accurately — record verbatim where possible
Ask the witness to review and sign their statement
Interview the supervisor(s) responsible for the area — separately from workers
Interview the injured person when medically cleared to do so
Phase 6: Root Cause Analysis
This is where most construction investigations fall short. Go beyond the immediate cause to organizational factors.

Build a timeline of events — what happened, in sequence, from normal work activity to the incident
Identify absent or failed defences: what controls should have prevented this but didn't?
Identify individual/team actions: what did people do or not do?
Identify task/environmental conditions: what made the task more difficult or more hazardous?
Identify organizational factors: what management decisions, system gaps, or resource issues created these conditions?This is the most important level — and the one most often skipped
Apply 5 Whys to each contributing factor — push past "human error" to systemic causes
Verify each causal factor with evidence — don't accept speculation as fact
Check: has this type of incident or near miss happened before on this site or within the company?
Consider whether this is a pSIF (potential serious injury or fatality) event requiring escalated analysis

For every causal factor, identify at least one corrective action that directly addresses it
Apply hierarchy of controls: prioritise elimination and engineering controls over administrative and PPE
Assign each action a specific owner (name, not just a role)
Set a realistic due date for each action
Define how completion will be verified — physical inspection, updated document, training record
Identify any immediate interim controls needed before permanent fixes are in place
Check if any actions have site-wide or company-wide implications — escalate if so
Enter all actions into your tracking system with owners and due dates
Phase 8: Report, Communication & Close-Out

Complete the formal investigation report — methodology, evidence, causal factors, corrective actions
Submit regulatory notification/report within required timeframe
Notify principal contractor and client as required by contract
Conduct a safety stand-down or toolbox talk to communicate learnings to the workforce
Share learnings with other sites or projects within the company
Update the SWMS or procedures affected by the investigation findings
Track corrective action completion — follow up with owners at due dates
Physically verify each action is implemented before marking complete
Conduct a follow-up review 30–60 days after close-out — has the risk actually been reduced?
Archive investigation records in a retrievable system — you'll need them for future trend analysis
Common Construction Incident Types — Special Considerations
Falls from Height
Falls are the leading cause of construction fatalities. When investigating, specifically examine: edge protection design and installation, harness inspection and anchor point adequacy, rescue plan (was there one?), and whether workers were working in proximity to unprotected edges as a routine practice or a one-off deviation.
Scaffold Failures
Check: scaffold design (was it engineered?), erector competency, inspection regime, modification history (was it modified after inspection sign-off?), and load management. Scaffold failures are almost always a system failure, not a random structural event.
Crane and Lifting Incidents
Examine: lift plan adequacy, exclusion zone management, load assessment, operator certification, signal communication, and ground conditions. The most common contributing factor is inadequate lift planning — particularly for complex or non-routine lifts.
Excavation Incidents
Investigate: soil classification, shoring/battering design, inspection regime, changing conditions (rain, vibration), and whether service locating was completed before excavation began.
Electrical Incidents
Focus on: isolation procedures, permit systems, cable detection, proximity to overhead lines, and whether electrical work was within the competency of the person performing it. Unauthorised electrical work is a recurring factor.
